This app provides medication tracking, blood pressure and blood glucose monitoring for kidney transplant patients at MUSC enrolled in the MITIGAAT study.

After patients log into the app using a personal identification number (PIN), they are taken to the app home screen. From there they can record and/or review medications, blood pressures, blood glucose values, and take a medication adherence survey. From this screen they can also call the transplant center, study coordinator, or pharmacy for refills and send real-time email alerts to the transplant pharmacist to document a medication change , an emergency room (ER) visit, or a hospital admission. When the patient taps the “Medications” button on the home screen, he or she can review the complete medication list, which is automatically updated from the EHR. In addition, patients can review their medication regimen and scheduled administration times and select which medications they are taking to document self-reported adherence.

If patients want to document medication adherence, they tap the medication adherence survey button on the home screen, which brings them to a survey for documenting adherence and barriers to such. To review and automatically upload (using Bluetooth syncing) blood pressures and blood glucose values, the patient taps the appropriate button on the home screen to access the blood pressure or blood glucose page. The data from these Bluetooth-connected home monitoring devices (iHealth system) is automatically synced to the mHealth app and transmitted to the web-based portal for the healthcare providers to review.
