The ankle-brachial index (ABI) is a non-invasive screening test with good sensitivity and specificity for detecting peripheral arterial obstructive disease (PAOD).
It is a preventive, simple, painless, complementary, blood pressure, non-invasive, easy to perform exam.
It is performed with the following equipment: sphygmomanometer (pressure device) and portable vascular Doppler device. The Ankle Brachial Index (ABI) exam serves as a prognosis for Peripheral Obstructive Arterial Disease – PAOD affected in the lower limbs of diabetic users. Progressive atherosclerotic obstruction of the arteries of the lower limbs gradually adversely affects the quality of life of patients.
Early diagnosis of PAOD offers a unique opportunity to act on the main risk factors and modify the cardiovascular profile, improving patients' quality of life.
The ABI calculation consists of evaluating systolic pressure measurements in the upper limbs (brachial artery) and lower limbs, at ankle height (posterior tibial and dorsalis pedis arteries). Each artery has its SBP assessed using Portable Vascular Doppler and two consecutive SBP measurements are obtained in each vessel.
The Index is calculated bilaterally using the following ratio:
Right ITB: highest SBP of the right ankle
highest mean brachial SBP
Left ABI: higher SBP of the left ankle
highest mean brachial SBP
Example:
If the user's right arm had SBP 122 and 126 mmHg, the average will be 124 mmHg and if the left arm had SBP 130 and 134, the average will be 132 mmHg. Thus, the value of 132 will be used in the denominator of both the left and right ITB, what will change will only be the numerator.
To carry out the exam, the user must follow the same recommendations as for arterial measurement, that is, not having a full bladder, not smoking or drinking coffee in the last 30 minutes, resting 5 minutes beforehand, not crossing arms or legs and not talking. in the procedure.
