As a continuation of this innovation trajectory, we present HEALTH FOR YOU, a Health Management application. In a single platform you register all your health information, obtaining as a result the survey of your current profile, possible related risks and alternatives and orientations to change living habits.
In the quest for active and successful aging, in view of the increase in life expectancy in the world, we understand as fundamental the empowerment of care and the possibility of having this opportunity in the palm of our hand.
To start your navigation follow the steps below:
1º) Download the Health for You application on the Play Store and App Store platforms
2º) Fill in your registration information
3º) Start filling out your Health Profile
4º) Finalize and visualize its Matrix of Risks and Diseases in Health
5) Save your Health Guidance Matrix and gain access to a guide to reach your goals, experiences, and support to change living habits
6) Feed your Health Diary and get benefits and access to services with discounts and advantages.
After completing the registration, you will be accompanied free of charge by the Health Management Team for a period of 30 days. After this period, if you are interested in maintaining the follow-up and interaction with our health team, you can hire one of our Programs. Otherwise, the application will remain available for use as a health journal, and you may at any time, contact our company or contract services.
Health Management for You - Services
* Health Profile Assessment: a user health survey that aims to track risks and aggravations in relation to their clinical condition, life habits and independence / vulnerability and risk of fall, as well as directing the appropriate actions and guidelines in Cheers.
* Health Support Center: composed of a multidisciplinary team in health, doctors, nurses, nutritionists and physiotherapists, which provide the transfer of health information and guidance in a safe and preventive manner, from Monday to Friday, from 8 am to 6 pm.
* Tele Orientation in Health: telephone contact by multidisciplinary team. Personalized service for guidance, encouragement and support in achieving goals and changing lifestyles.
* Messaging service: sending SMS text messages with personalized health tips and guidelines according to the risks identified in each user, as well as reminders of medical consultations, exams and medication administration. Messages are sent in the form of friendly, interactive conversations. It allows monitoring behaviors, responses and health information in general.
* Contact Us: communication channels with the team through online chat and email.
* Health Videos: according to the risks identified, the user will have access to educational videos.
* Network of resources: access to the network of partners and service relationship in health, leisure and well-being.
* Individual services in health: you can request the contracting of health services as safety devices for fall prevention, emergency pushbuttons, medication control, home and outpatient visits with multidisciplinary team (doctor, nutrition, physiotherapy, nursing) , nursing procedures, medical and hospital equipment and materials, enteral diets and dietary supplements.
